How you will make an impact:
As the MDS Coordinator at St. Mark Village, you will be a Registered Nurse who will oversee the Resident Assessment Instrument (RAI), care planning, and PDPM processes to ensure compliance with regulatory standards and support optimal resident care. Key responsibilities include conducting and reviewing MDS assessments, managing OBRA and Medicare schedules, supporting interdisciplinary care planning, and participating in utilization review and quality assurance. This role requires strong clinical oversight, collaboration with team members across all shifts, and a commitment to maintaining high standards in resident well-being, documentation accuracy, and regulatory compliance.
